So I'm writing a book that you can see on my universes called Kìshado's Disease and Levi who makes his first appearance in chapter 2, his character is confusing… He lost his mother/family after their cruise sank and, his mom called Levi he/him and so will I. But… in reality, Levi doesn't pay attention because he never learned about pronouns, he was just always called he/him so he didn't care. So when his friends refer to him, they call him they/them or something else but never he/him. So… if the narrator (me) calls Levi he/him but the people in the book call Levi they/them, will that be to confusing?

so if I'm understanding correctly then Levi is a he him but his friends call him they them?

It might be helpful to explain in the narrative or through dialogue why Levi's friends refer to him using different pronouns than the narrator uses. This can provide context and clarity for readers and avoid any confusion. Additionally, consider having the narrator consistently use the pronouns that Levi prefers, rather than the ones his friends use, to reflect his true identity and show respect for his choices, but it sounds like you're doing that already. In all, as long as you acknowledge it at some point with some explanation, I don't think it'd be too confusing to readers.
