Okay so, Pato I will go find that channel and bring back the name to you but first Ima answer Mosis' question.

Mosis, she did not do it to herself. She had a disease, and it was clearly a degenerative one. That means she ultimately would've died sooner or later. Even though Kaori was scared of death, as shown in that scene where she was eating caneles with Kosei on the roof of the hospital, she was willing to take that one in a million chance of surviving the surgery. Kosei had said he wanted to play with her again, and so that was yet another incentive to take the surgery. It's a sad, but really good ending to the anime.
