She appears as a sickly looking woman with acidic slime for hair dripping all around her. Her eyes are pure black. She wears mismatched necromantic robes that barely cover her.

Anathexia has the ability to create and bestow afflictions at will to any mortal she pleases. She does this as a reward usually and as a sign of her favor. She also maintains the powers most infernals possess such as teleportation, shapeshifting, and so on.

She interacts with mortals directly and through messengers, mentally and physically. Sometimes, she appears through a vessel like a slug, snake, or afflicted beggar.

To show their dedication, followers get together in a hideous ceremony known as the Afflicted Rite. During this ceremony, they engage in all manner of taboo: mortal sacrifice, exchanging afflictions, and even cannibalism.
