Hubris, medical knowledge weakens him, certain artifacts, mage slayers, God Slayers (a very specific weapon in the shape of a spear)
"lessened", due to his war with Arthur Greykeep, Nezarath was barred from interfering with mortals in any direct manner and would not be allowed to bend that rule for an epoch, so Nezarath did the only thing he could and awoke his mortal Champion Canis Vance from his imprisoned grave and he pumped a large amount of his power into him, allowing him to technically not break the rules

History
the loss of his mortal champion - some point in either the first or mythic age Zezarath's mortal champion is slain, his body shoved in a blackstone casket, stabbed through with 17 ceremonial Burial Blades, and shoved into a volcano
the first "god war" incident - an ever escalating attempt by Nezarath to rid the world of Arthur Greykeep a foreign body to this land and one who's alternate reality double almost entered the universe with an army capable of taking over turns sour when the demons he'd contracted from Bowlog accidentally kidnap Arthur's romantic partner instead resulting in Greykeep calling in some favors and bringing an army equipped with powered Karpachi exo-armors and weapons specifically designed to kill him to his realm for a fight, it did not go well, even with Greykeep's forces initially losing large numbers, he eventually got to Nezarath's main home and put it to siege, Nezarath had to sue for peace with Tal' Zanithal over seeing and Nezarath was left metaphorically penniless afterword unable to directly interfere with the mortal realm, and he couldn't bend that rule, not with Tal' Zanithal watching him
retrieving his lost champion - in attempt to regain his ability to interact with the mortal world, Nezarath brokers a deal the Hollow Holm dwarves, they retrieve the casket of his champion and he gives them some of the God Slayer spears that fool Greykeep left in his realm, they agreed and even gave Nezarath some extra for helping advance some of their research that had stalled

Nezarath is not just the god of plagues, he's also a god of inevitability, the curse of aging, and death
the something extra the dwarves gave him was the same weapon used on Zamanar during her escape but several iterations later, and much more deadly
